I UNDERSTAND THAT BAY AREA CHAMPIONS IS A SMALL-SIDED SOCCER LEAGUE FOR ADULTS, AND BY SIGNING THIS WAIVER, I ACKNOWLEDGE THAT MY CHILD (OR MINOR UNDER MY GUARDIANSHIP) IS PARTICIPATING WITH MY FULL CONSENT AND UNDERSTANDING OF THE ASSOCIATED RISKS. I AGREE TO RELEASE, INDEMNIFY, AND HOLD HARMLESS BAY AREA CHAMPIONS, ITS ORGANIZERS, COACHES, REFEREES, AND AFFILIATES FROM ANY LIABILITY FOR INJURIES, DAMAGES, OR CLAIMS ARISING FROM PARTICIPATION IN THE LEAGUE. I ALSO AFFIRM THAT MY CHILD IS IN GOOD PHYSICAL CONDITION TO PLAY SOCCER AND THAT I ASSUME ALL RESPONSIBILITY FOR ANY MEDICAL EXPENSES INCURRED AS A RESULT OF PARTICIPATION.
I HAVE READ THE WAIVER AND RELEASE OF LIABILITY FORM BELOW CAREFULLY AND UNDERSTAND ITS TERMS. I ACKNOWLEDGE THAT I AM GIVING UP SUBSTANTIAL RIGHTS BY SIGNING THIS DOCUMENT, AND I SIGN IT KNOWINGLY, INTELLIGENTLY, AND VOLUNTARILY WITHOUT ANY INDUCEMENT OR DURESS. I UNDERSTAND THAT BY JOINING ANY OF BAY AREA CHAMPIONS LEAGUES I HAVE TO AGREE AND SIGN THE PLAYER WAIVER, WHETHER I AM A TEAM CAPTAIN, PLAYER IN A TEAM, OR FREE AGENT. I ALSO ACKNOWLEDGE THIS WAIVER IS VALID FOR ALL LEAGUES FROM JANUARY 1ST 2025 TO DECEMBER 31st, 2025. (copy)

This is the Bay Area Champions League participant waiver that all players must agree to before participating in any of our leagues.

A signed waiver is valid for all leagues from January 1, 2025, to December 31, 2025.

In consideration of being allowed to participate in any way in any programs or leagues hosted by Bay Area Champions at The Art of Football LLC and/or any of its subsidiaries or affiliates, I understand, acknowledge and agree to the following:


1. Physical Fitness and Safety:
I understand the physical requirements of participating in the Bay Area Champions soccer league and certify that I am physically fit and capable of participating. I have not been advised otherwise by a qualified medical professional. I agree to comply with all rules, regulations, terms, and conditions of the league, including the rules of soccer and the Participant’s Code of Conduct. I will inspect the equipment, facilities, and premises before participating and will notify Bay Area Champions officials, team captains, referees, or facility owners if I find anything unsafe. I will refuse to participate if any safety concerns are not addressed.

2. Assumption of Risk:
I acknowledge and fully understand that participating in the Bay Area Champions league involves risks of serious injury, including permanent disability and death, as well as property damage and significant social and economic losses. These risks include, but are not limited to, those caused by: (a) the actions, inactions, or negligence of Bay Area Champions, other participants, volunteers, spectators, coaches, event officials, referees, and organizers; (b) the condition of the premises, facilities, or equipment used; (c) the rules of play; (d) weather conditions; (e) the skill, ability, and condition of other participants, and their failure to adhere to the rules and regulations; (f) potential exposure to infectious diseases; and (g) vehicular and pedestrian traffic. I further understand that there may be other risks not known or foreseeable at this time. Bay Area Champions cannot control these risks and has not evaluated my skill level or ability prior to my participation. Consequently, Bay Area Champions is not responsible for my personal health or safety during my participation in the league. I KNOWINGLY, INTELLIGENTLY, AND VOLUNTARILY ASSUME ALL SUCH RISKS, BOTH KNOWN AND UNKNOWN, ANTICIPATED AND UNANTICIPATED, FORESEEABLE AND UNFORESEEABLE, EVEN IF ARISING FROM THE NEGLIGENCE OF BAY AREA CHAMPIONS OR OTHERS, AND I ASSUME FULL RESPONSIBILITY AND LIABILITY FOR MY PARTICIPATION.

3. Responsibilities Regarding COVID-19 and Other Infectious Diseases:
I agree not to participate in Bay Area Champions activities if I am feeling unwell or if I am subject to quarantine or isolation as determined by local health authorities or the Centers for Disease Control. I will adhere to any health prescreening protocols set by Bay Area Champions and report the results before attending any event. I understand and agree that Bay Area Champions may be legally required to inform government health officials of any positive test results or potential exposure, and I consent to my name, contact information, and exposure status being disclosed as necessary. I will cooperate with any contact tracing efforts deemed necessary by Bay Area Champions or public health officials.

4. Waiver and Release of Liability:
On behalf of myself, my heirs, beneficiaries, executors, administrators, personal representatives, and assigns, I HEREBY WAIVE, RELEASE, DISCHARGE, AND AGREE NOT TO SUE Bay Area Champions, its affiliates, subsidiaries, parent companies, managers, members, directors, officers, employees, contractors, volunteers, representatives, agents, coaches, referees, other participants, sponsoring agencies, sponsors, advertisers, and owners or lessors of equipment and facilities used in the league (collectively the “Releasees”), from any and all claims for damages, injuries, losses, liabilities, and expenses, including those arising from any Releasees’ negligence, related to my participation in any Bay Area Champions events.

5. Indemnification:
I agree to INDEMNIFY, DEFEND, AND HOLD THE RELEASEES HARMLESS from and against any and all claims for damages, injuries, losses, liabilities, and expenses incurred by Bay Area Champions resulting from my participation in the league.

6. Medical Treatment Consent:
In the event of injury, accident, or illness during any Bay Area Champions event, I consent to receive medical treatment deemed advisable. I release Bay Area Champions and all persons involved in such treatment from any responsibility for these actions.

7. Media Release:
I grant Bay Area Champions and its authorized representatives the right to photograph, videotape, or otherwise capture my likeness, voice, and sounds (“Content”) during my participation. I acknowledge the Content as works made for hire and irrevocably assign all rights to Bay Area Champions. Bay Area Champions may use or sublicense the Content, along with my name, likeness, and biography, for promotional purposes without further consideration.

8. General Provisions:
This Waiver and Release of Liability is intended to be interpreted broadly to provide a release and waiver to the maximum extent permitted by applicable law. If any portion of this waiver is found invalid, the remaining provisions shall continue in full force and effect.

9. California Residents:
For participants in California, I knowingly waive all rights under California Civil Code Section 1542, which states: “A general release does not extend to claims that the creditor or releasing party does not know or suspect to exist in his or her favor at the time of executing the release, and that, if known by him or her, would have materially affected his or her settlement with the debtor or released party.”
